Pre-registration
If you are planning to have a baby at St. Joseph's Hospital or
Holy Cross Hospital, you may pre-register. Simply complete the
secure on-line pre-registration form. Pre-registering will speed
your admission to the hospital.

If you prefer, you may download and print the
Maternity Pre-Admission Form and mail it to the appropriate
admitting office address listed at the bottom of the form.

Great Expectations Package
Carondelet Health Network’s “Great Expectations Package” is
designed to assist expectant mothers who do not have insurance
coverage, or those without maternity coverage in their insurance
policy.
