Community Benefit Program

Faith Community Nurse Network

More than 1,000 people annually receive health services at schools, health fairs and other community events through the Faith Community Nurse Network. It serves communities with large populations of the uninsured and underinsured who experience barriers to access health care.

Carondelet Health Network provides the location and coordination for this network of over 50 nurses (some pictured here) around Southern Arizona.

Services include:

  • Physical, spiritual and psychological assessments
  • Personal counseling
  • Diabetes and general health education
  • Early disease detection and health screenings
  • Behavioral health promotion
  • Referrals to medical homes
  • The Faith Community Nurse Network also has collaborations with Santa Cruz Elementary School and San Miguel High School.

Weekly school visits provide:

  • Free immunizations
  • Blood pressure
  • Hearing and vision screenings
  • Other health education services
