Patient Rights & Responsibilities
Every patient at a Carondelet healthcare facility, including the patient’s legally authorized representative or parent/guardian for minors, shall have the right to: - Be informed of your rights
- Be treated in a dignified and respectful manner
- Receive information in a manner you understand
- Interpretation and translation services as needed
- Personal privacy
- Have your cultural and personal values, beliefs and preferences respected
- Religious and other spiritual services
- Access, request amendment to, and obtain information on disclosures of your health information in accordance with law and regulation
- Give or withhold informed consent for the care, treatment and services you receive, including consent to produce or use recordings, films, or other images for purposes other than care
- Keep and use personal clothing and possessions per facility policy
- Telephone and mail service (if appropriate)
- Access protective and advocacy services
- A safe environment
- Be educated regarding your diagnosis, medical condition, treatments and medications
- Participate in decisions involving your care, treatment and services
- Appropriate assessment and management of pain
- Accept medical care or refuse treatment to the extent permitted by law and be informed of the medical consequences of such refusal
- Have a surrogate decision maker to make decisions about your care, treatment, and services on your behalf if you are unable to make decisions for yourself
- Be informed (or have your surrogate decision maker informed) by the licensed independent practitioner responsible for managing your care about outcomes of care, treatment and services, especially those that you need in order to participate in current and future healthcare decisions
- Formulate Advance Directives and have facility staff and practitioners comply with these directives, in accordance with law and regulation
- Have your family involved in care treatment and service decisions to the extent permitted by you or your surrogate decision maker, in accordance with law and regulation and facility policy
- Make informed decisions regarding participation in clinical research
- Be informed of the facility’s complaint resolution process. You have the right to voice your complaints and recommend changes freely without being subject to coercion, discrimination, reprisal, or unreasonable interruption of care. You have the right for your complaints to be reviewed and addressed as appropriate
- Be informed of the reason for transfer to another facility
- Be informed of the names and titles of the individuals responsible for and providing your care, treatment and services
- Freedom from restraints and seclusion unless clinically necessary
- Freedom from all forms of abuse or harassment
- Explanation of hospital bill and financial counsel.
Patient Responsibilities - Along with your rights as a patient come responsibilities to ensure that you receive the highest quality health care that you deserve. Every patient and his/her representatives are responsible for:
- Communicating honestly and directly with your healthcare team and providing accurate information
- Cooperating with your healthcare team and following their instructions to optimize the care, treatment and services provided
- Asking questions and informing your healthcare team when you do not understand information or instructions and your treatment course or care decision.
- Participating in your medical plan of care
- Following facility rules and regulations
- Being considerate of fellow patients
- Being respectful and considerate of others and of facility property
- Informing the facility of violations of patient’s rights
- Reporting concerns regarding care and safety to facility staff
- Asking questions and understand how to meet your financial commitments
- Accepting the consequences for not following instructions, hospital’s rules and regulations, or any of the above responsibilities.
