TUCSON, Ariz. – (April 21, 2010) – Carondelet Health Network is proud to announce that Donna Zazworsky, RN, MS, CCM, FAAN has been named Vice President of Community Health and Continuum Care.
In her new role, Zazworsky will lead the continuum efforts of the many Carondelet programs and services related to access to health care, health promotion and disease management. These include the Southern Arizona Health Village for the Homeless, Faith Community Nursing, Diabetes Care Center, the Carondelet Diabetes Education Institute and Telemedicine programs.
“I meet people throughout the community every day that are genuinely excited and proud to make a difference,” said Donna Zazworsky. “Carondelet has a unique opportunity to help people, many of them in desperate need of healthcare and healthcare education, and this role has given me the resources to do just that.”
Zazworsky will develop and implement a community case management system for Carondelet. In addition, she is leading Health, Human Services and Faith-based Workgroup for the newly awarded $16 million Centers for Disease Control Obesity Prevention grant to Pima County.

Zazworsky has extensive experience in diabetes education and Telemedicine programs. She has created programs that deliver clinical and educational diabetes services to rural hospitals, targeting at-risk populations.

Zazworsky earned her BS in nursing at Pennsylvania State University and her MS in nursing with a specialty in community health nursing at the University of Arizona. Among the many awards she has received include the Carondelet Health Network President’s Award in February 2010; Women of Influence: Inside Tucson Business, 2009; Leader to Watch: Nurse Leader Journal, April 2009; University of Arizona Alumni Award, 2008; and an American Diabetes Association of Southern Arizona Care Award, 2007.
