Accessibility Statement

We are committed to providing a website that is accessible to the widest possible audience. To do so, we are actively working with consultants to update the website by increasing its accessibility and usability by persons who use assistive technologies such as automated tools, keyboard-only navigation, and screen readers.

We are working to have the website conform to the relevant standards of the Section 508 Web Accessibility Standards developed by the United States Access Board, as well as the World Wide Web Consortium's (W3C) Web Content Accessibility Guidelines 2.1. These standards and guidelines explain how to make web content more accessible for people with disabilities. We believe that conformance with these standards and guidelines will help make the website more user friendly for all people.

Our efforts are ongoing. While we strive to have the website adhere to these guidelines and standards, it is not always possible to do so in all areas of the website. If, at any time, you have specific questions or concerns about the accessibility of any particular webpage, please contact so that we may be of assistance.

Thank you. We hope you enjoy using our website.

Request Medical Records

Accessing Your Patient Medical Records

Are you looking for copies of your medical records or test results? Carondelet Health Network is happy to help you obtain your medical records from our locations.

You can access your inpatient medical information online through our patient portal.

Process for Obtaining Records

To obtain your records, you or a representative must complete an “Authorization for Disclosure of Protected Health Information” form. You can find these forms by asking your nurse, the concierge desk or at a Health Information Department (HIM) listed below.

Click here to download the "Authorization for Disclosure of Protected Health Information" in Spanish.

Your completed and signed form can be submitted in person, faxed or scanned and mailed to the HIM Department. Unfortunately, we cannot accept an email without the scanned document showing your signature and date.

You also have the option of faxing or mailing a signed and dated letter to the HIM Department requesting your records instead of completing the request form. In this request letter, you must state the:

  • Patient name
  • Birth date
  • When care was received
  • What records you’re requesting
  • The purpose of requesting the records
  • Permission to release any information, including (if any) information about drug/alcohol/behavioral health or HIV
  • Address where the records should be sent

If you’re requesting records and are not the patient, the letter must state your relationship to the patient, such as “patient’s parent.” If you are not the patient and/or the patient is not a minor, you must include a power of attorney.Records Needed for Care

If your records need to be sent to your doctor for follow-up care, you can include the doctor’s name and contact information on your authorization form. If your doctor needs copies of your medical records right away, ask his or her office to call our HIM Department for copies.

Records for Personal Use

If your medical record copies are for personal use, there may be a charge for photocopying a large number of records. If there is a charge, we will contact you with an estimated cost of completing your request for your medical records.
